MEMBERSHIP FREEZE POLICY

ATTIRE

LifePlex is a membership club. All guests must sign in at the member services desk, pay a $20.00 guest fee ($15.00 if accompanied by a member). IHRSA member fee $15.00 with validated proof of membership. Every guest must provide a photo I.D.
Short term memberships are available, please contact the membership department.

A complimentary guest pass may be provided at the discretion of the membership coordinators. Members in good standing may inquire about bringing in a complimentary guest provided the following criteria is met.

· All guests must be 18 years of age or accompanied by a parent or

guardian.

· All requests must be made at least 24 hours in advance.
· All complimentary guests must live within a 15 mile radius.
· All guests must fill out a club guest waiver in it's entirety and

meet with a membership representative.

· Guests may only use a complimentary pass once.
· First time visitors only.

A member in good standing may freeze his/her annual membership for medical reasons only. A membership freeze will become effective with the club’s medical committee approval of a valid physician’s note. The freeze time will begin on the date the note is delivered to the club. No retroactive time credit will be granted. The minimum freeze time per note is one month. The maximum freeze time per note is three months. Administrative fees and or co-pays apply to Discounted memberships.

Proper attire, including sneakers, is required by members when on the fitness floor; therefore, the following is prohibited: cut-off denim shorts, open toed shoes, shoes, pants, belts, jeans, cut-off shirts, and tank tops that are not full fitting. Any attire or behavior which may be considered offensive or too revealing to the general public is prohibited. The clubs definition of offensive and or revealing is final.

Behavior

LifePlex is a health club where members are expected to be courteous to one another and to our staff, and to refrain from creating excessive noise and using profanity. Proper etiquette is required and expected at all times. All equipment and accessories should be used within the manufacturers guidelines. At no time is it acceptable to drop, toss or throw any weight or piece of equipment. All members are required to lift weights and operate the fitness equipment within their lifting/weight capabilities so that you are in control of the weight/equipment at all times.

YOUR MEMBERSHIP CARD

We ask that members treat our facility with the same care as they would their own homes.  Therefore, the following rules apply:

* keep the level of noise to a minimum
* clean sink area after using (e.g., shaving)
* be responsible for personal cosmetics
* wring out bathing suits only in wet area
* place wet bathing suits in plastic bags provided
* do not hang bathing suits on corners of lockers
* do not use toilet stalls for changing purposes

* do not leave any apparel in sauna area

For security purposes and to protect your rights as a member, we will be very strict with our check in procedure. The Ramapo Police Department recommends our checkin policy be enforced and photo ID to be part of our process at all levels.  Therefore, you will be required to stop and present your membership card each time you come in to the club.  This procedure will also serve to expedite your entry into the club.  If a membership card is lost, the member is responsible for purchasing a replacement card. Your card is also required for participation in our free towel service.

Included in a new regular annual pre-paid membership are three Personal Training sessions. Our personal trainers will help you begin a weight training and or cardiovascular program designed for you. Normal policies and procedures of apply, including the  Cancellation policy.
